• Change of Status form
  For students completing their diploma through the Independent Study High School, fill out a Change of Status form if you need to update your mailing address or your proctor's name or address. This form may be saved and submitted by e-mail or postal mail.

  Independent Study High School diploma students preparing for graduation must complete the following information to receive their diploma.
  
  
• Lesson wrapper
  Use lesson wrappers when you need to mail in an assignment.
  
  
• Local School Approval form
  If you are planning on transferring credit to your local school or learning organization, you’ll need to seek prior approval from your school. This form may be saved and submitted by e-mail or postal mail.
  
  
• Scholarship Application
  The K.O. Broady Scholarship Fund provides financial assistance to students. Application deadline is October 31.
  
  
• Student Record Release Form
  Use this form to authorize the release of student grades and transcripts.
  
  
• Transcript Evaluation form
  If you would like to earn your diploma from the Independent Study High School and have earned existing high school credit, please complete a Transcript Evaluation Form. You will also need to submit official transcripts from all previous high schools, to receive an adviser's evaluation of your transferrable credit hours.
  
  
• Transcript Request form
  Submit the Transcript Request form to have an official transcript from the University of Nebraska’s Independent Study High School generated for you.
